**Activate Physiotherapy Corporate Membership Program – Terms and Conditions**

Thank you for considering Activate Physiotherapy for your health needs. Our initial consultation offer of $60 for the Corporate Membership Program is designed to provide your employees with personalised and comprehensive physiotherapy services. Please carefully review the following terms and conditions before proceeding:

Details:
The offer entitles employees of organisations in the WhereFit Corporate Network to receive an initial consultation of 30mins duration.

Eligibility:
This offer is exclusively available to employees of organisations in the WhereFit Corporate Network.
The offer is valid once per eligible person

Appointment Scheduling:
Appointments must be scheduled in advance by booking online or calling your nearest clinic. Activate Physiotherapy will make reasonable efforts to accommodate preferred appointment times, subject to availability.

Payment and Invoicing:
The $60 initial consultation fee is payable at the time of scheduling the appointment.
Employees covered by Private Health insurance with extras may also be eligible to claim a benefit for physiotherapy services.
The offer is not redeemable for cash.

Cancellation and Rescheduling:
In the event of cancellation or rescheduling, please provide at least 24 hours’ notice. Failure to do so may result in the forfeiture of the initial consultation fee.

Offer Duration:
The Corporate Membership Program initial consultation offer is valid for a limited time. Activate Physiotherapy reserves the right to modify or terminate this offer at its discretion.

Confidentiality:
All information shared during the consultation will be treated with the utmost confidentiality in accordance with Australian privacy laws (Privacy Act 1988).
